For years, HealthPoint CHC had been relying on an on-premises platform to manage and adjust its data. The health system needed the ability to access patient and organizational data at faster, more detailed levels to improve decision-making and care. “Our operations leaders need to know [if our] services are equally meeting the needs of all of our patients,” says Kathrina Sharman, VP of operations and patient experience. “The ability to understand our patient experience can’t be coming 90 days later. It needs to be coming now.”
Pivoting to a robust cloud data estate for easy data access & analysis
Recognizing the need for a cloud-first enterprise data warehouse to overcome roadblocks, Insight proposed a comprehensive solution including both strategy and execution. This involved full Azure® cloud implementation via our teams, leveraging Databricks as the primary platform for data processing.
The solution would enable seamless access to data from more than 10 business-critical sources, including Electronic Medical Records (EMRs), Electronic Health Records (EHRs), pharmacy data, claims data and more. Additionally, adopting a true lakehouse architecture with an SQL warehouse that incorporates Protected Health Information (PHI) and Personally Identifiable Information (PII) handling would enable secure, controlled data access for users with varying levels of permissions.
Becoming data driven through Organizational Change Management (OCM)
HealthPoint CHC also engaged Insight’s OCM services throughout the entire project to facilitate transformation. Insight’s OCM experts assessed HealthPoint CHC's existing OCM capabilities, evaluated potential risks and identified the impacts of change.
Additionally, OCM has facilitated the collection and measurement of adoption rates, enabling HealthPoint CHC to gauge the success of its data-driven initiatives. And by incorporating resistance management techniques, OCM ensures a smoother transition and alignment of sponsorship and leadership. Through these efforts, HealthPoint CHC has successfully positioned itself as a data-driven organization, reaping the numerous benefits that come with making data-backed decisions and enhancing their overall performance.
The outcome: A revolutionized, cloud-first approach to healthcare with data insights
The implementation has been transformative for the health system. With a new data ecosystem in place, teams can now quickly access data in a presentable and curated format. This enables business stakeholders to monitor key metrics easily, empowering the client to make data-driven decisions, understand patients at a deeper level and ensure the delivery of timely and high-quality care.
HealthPoint CHC’s new stratified data approach also helps address disparities in patient care. For example, knowing which diabetic patients don't have their condition under control helps providers respond faster and in more personalized ways. HealthPoint CHC is also now able to pinpoint gaps in care and the inequities that affect specific patient populations — from refugees to communities of color and beyond.
Moreover, leveraging financial data is crucial for long-term fiscal sustainability. Analyzing the total cost of care and services provided at other sites allows HealthPoint CHC to make more strategic decisions about expanding to new locations or additional access points. Lastly, by leveraging its improved data ecosystem and meeting quality care metrics, HealthPoint CHC increases its chances of securing the necessary funding to support vital initiative.
